Our office is in-network with BCBS, Cigna, United Healthcare, and a NON-PAR provider for Medicare. We are continuously looking at other medical insurance networks to join as well. We also have a non-insured treatment program available to those approved patients. Even though Medical insurance is a private arrangement between you and your company, we will file your claim, handle all the paperwork, and help you get maximum YOUR insurance benefits.
We accept cash, personal checks, and most major credit cards. If paying by cash, ask about our “Cash Discounts” program. Ask us about our easy financing options with Care Credit, an interest-free, flexible monthly payment plan you can apply for right in our office.
You will be greeted by one of Dr. Davis’s friendly staff members, who will have you complete any necessary forms for your consultation along with insurance requirements, if you haven’t already filled the below forms out ahead of time.
We would also appreciate it if you brought your sleep study (if applicable), copy of Panorex or Cone Beam scan (we will take a new Cone Beam scan if you do not have one), along with any and all referrals from Sleep physician or primary care.
Our assistant will escort you to your room where you and Dr. Davis will review your sleep study and address your needs. If you proceed with the device that day, our assistant will then go over the consent forms and paperwork with you. Dr. Davis will take a scan impression of your teeth and will send them to the lab to fabricate the device. After the consultation and x-rays, we are ready to get you checked out and scheduled to return in roughly 2-4 weeks for delivery of your new device and home care instructions.
